Faribault Care Center is a 55 bed skilled nursing and rehabilitation center. Dementia and Behavioral Unit

Faribault Care Center has 13 beds in our memory unit. Our specialized secured living space and trained care team help seniors live safely and comfortably with the medical care they need. Our goal is to allow our residents to live their lives to the fullest.  This is accomplished by taking the time to individually understand where each resident's reality is at and meeting them there.  We inquire about resident’s personal preferences, offer choices, respect privacy, and treat residents in a dignified manner.  We will work to keep residents as independent as possible, and encourage participation in decision making to the extent possible. Our focus is on maintaining high standards of care, while enhancing one’s quality of life.  We provide meaningful interaction on a daily basis.

Our multidisciplinary team consists of physician, nurse practitioner, nurses, nursing assistants, therapy, dietary, social worker and activities’ staff that are dedicated to these residents and their individual needs. We conduct comprehensive, pre-admission assessments to ensure that each resident will receive the individualized care plan necessary for successful residency at our facility..
